Pharmaceutical Horizons’ Case Management services provide plan support from individual patient evaluation to disease or clinical condition assessments for plans or groups of physicians. Our program includes five scheduled patient specific utilization reviews each month. Additional reviews can be included on a fee for service basis.

For organizations that maintain their own case management nurses/staff, our service is available on request.

For Medical Case Managers who need patient profiles, we can provide the following information:

• Drug duplication

• Drug dose problems – patients not taking medications as prescribed (a compliance/adherence problem)
